All Local 1445 members are eligible for this free health evaluation,
regardless of where you work!

To receive your free personal health assessment (PHA)
you will need to register at this website, sponsored by Blue Cross/Blue Shield of Rhode Island.

The registration form will look like this:

• First, under the "New Users" section, create your personal username and password (your password must be at least 6 characters long and include at least one number), then reenter your password to verify it, and provide your email address. (Make a note of your username and password so you can log in again later.)
• Then, in the campaign code box enter UFCW7.
• Finally, click on the "Create Account" button.

Now you're ready to complete the health assessment.
 
Answer all the questions and upon completion you'll receive your online PHA report, which will include a personalized health risk report with tailored resources and action steps. It's that simple! And it's free for all Local 1445 members!
